Nurse-Led Intake Visit
A seasoned nurse guides the intake, gathers detailed information, and helps determine the appropriate level of care, and whether skilled or unskilled home-healthcare services are required.
At this stage, a physical medical assessment cannot be performed. PA DOH regulations require physician approval first. The full clinical assessment happens in step 4A.
We Design the Right Care Model
Skilled nursing, elite caregiving, or a hybrid, built specifically for your situation.
Skilled Nursing & Therapy
We coordinate directly with your primary care provider and/or attending physicians to obtain required medical orders. Once approved, an RN, OT, or PT clinician visits for the in-person assessment. A formal Plan of Care is then written and sent to the physician for signature.
No additional fee for this process.
Elite Caregiving
A personalized care plan focused on daily living support, companionship, and concierge-level services. Ensures safety, comfort, and quality of life. No physician orders or in-person nursing assessment required.
Hybrid Care
Combines skilled nursing or therapy to address medical concerns with elite caregiving for daily living and companionship. Requires all steps from 4A and 4B. Skilled care is delivered only when a physician orders it; caregiving provides consistent comfort, safety, and continuity throughout.
